Setting up Terminal Services

I need step by step instructions on setting up Term Services. Never worked with it before. Doctors Office with 2 servers; Main server is DC, DNS, DHCP and SQL DB, 2nd server is for Term Services only. Both servers are 2003 R2/SP2. I need to know what ports need to be opened in the firewall, will these ports will point to the IP of Term Server? User configuration, software setup and configuration, Will RDP be used from the remote clients? Term Server configuration and what other configuration will be needed.
